A Form in Ultra is like a Blackboard Test but with fewer question types and with the addition of the of rating questions in the form of Likert question type. They can be used as a survey tool, alternatively you can switch it to a graded form and use it in an assessment.<\/p>\n<ul>\n<li><a href=\"#Section1\">Create a Form<\/a><\/li>\n<li><a href=\"#student\">Student view<\/a><\/li>\n<li><a href=\"#results\">View Form results<\/a><\/li>\n<li><a href=\"#mark\">Form as an assessment<\/a><\/li>\n<\/ul>\n<h2 id=\"section3\"><a id=\"Section1\"><\/a>Create a Form<\/h2>\n<p>Go to the place in the course content where you would like to insert the Form and hover over the grey line until the purple plus sign icon appears.\u00a0 This will open the Add Content Menu, select Create, the Create Item panel will open on the right-hand side of the screen.<\/p>\n<p>In the Create Content panel select &#8220;Form&#8221;.<\/p>\n<p><img decoding=\"async\" loading=\"lazy\" class=\"alignnone size-full wp-image-19952\" src=\"https:\/\/sites.reading.ac.uk\/TEL-Support-Staff\/wp-content\/uploads\/sites\/92\/2024\/03\/Form-1.jpg\" alt=\"Screenshot of Create Item panel with Form option highlighted. \" width=\"311\" height=\"281\" \/><\/p>\n<p>Once you have selected Form the create From page will open.<\/p>\n<ol>\n<li>Add a title to your Form<\/li>\n<li>Add content to the Form\n<ol style=\"list-style-type: lower-alpha\">\n<li>Use Add Text option to add instructions using the Ultra text editor, or Add local file to attach instructions\/information.<\/li>\n<li>Add questions from the list of question type options\n<ol style=\"list-style-type: lower-roman\">\n<li>Essay Questions<\/li>\n<li>Likert Questions<\/li>\n<li>Multiple Choice Questions<\/li>\n<li>True\/False Questions<\/li>\n<\/ol>\n<\/li>\n<li>Add Page Break if required.
